Okay, here's the pattern:
All items receive their new slots on these upgrades except for rings and amulets which get their first slot without any upgrades. However, the maximum number of slots depends on the item type.
Capes never get any slots.
Boots, gloves, rings, daggers, and quivers have a maximum of 2 slots.
Helmets (including hats and mage brims), greaves, and amulets have a maximum of 3 slots.
Shields and all weapons except for assassination daggers and mage staves have a maximum of 4 slots.
Armor cuirass has a maximum of 5 slots.
Mage robes and mage staves have a maximum of 6 slots.
